Customer Training Enrolment Form - For In-house Training Program

• No. of seats offered to each customer will depend on the product/software.(Refer to related course agenda)
• For new installation, the training is offered to customer free for one time under each FSMA contracted machine within 6 months after the installation date. For refresh training, training is offered free within the 1st year after the installation date.
• Customer Training will contact customer to confirm booking.
• The deadline for enrolment is 2 working days (New Installation & Software operation training) and 3 working days (Refresh Training) before the actual training day.
• Please make copies if necessary.
